Home Health Care

With a proud history that includes pioneering efforts in establishing home care as an alternative nursing care setting in long-ago San Francisco, UCSF Home Health Care today finds itself the largest non-profit provider of home care services in San Francisco. Our ethnically diverse staff mirrors the Agency’s ethnically diverse patient population. Staff provide culturally sensitive services with bilingual case managers available. The Agency programs range from maternal/child health and pediatric to adult.

UCSF Home Health Care supports the UCSF Medical Center’s mission of caring, healing, teaching, and discovery by providing comprehensive, coordinated, and compassionate home care services to patients and their families. The Agency actively pursues opportunities to forge cutting-edge practices and clinicians are encouraged and supported to advance in their practice specialties. Two successful areas of Performance Improvement focus have been complex Wound Management and Diabetes Management. Both of these Agency specialties have identified clinical nurse resources and both projects have culminated in Medical Center Distinguished Nurse awards for the clinician participants.

The UCSF Home Health Care clinician culture can be described as collegial, hard-working, and well-resourced. The multidisciplinary staff work closely in coordinating and delivering high quality care. Geographically separated from the Medical Center, the Agency nonetheless has access to and makes ready use of multiple clinical specialists from the in-patient side. At the same time, national nursing leaders on faculty at the distinguished UCSF School of Nursing continue to make themselves available as consultant resources over a wide-ranging set of clinical challenges. Clinicians feel well supported and have multiple resources to turn to for problem solving. At the same time, staff actively participates in regular, multi-disciplinary case conferences, where complex patients are discussed and all disciplines including Pharmacy and the Agency’s Medical Director take part.

UCSF Home Health Care patients continue to be very satisfied with their care. The Agency receives steady high marks on satisfaction surveys and frequent written feedback from patients and families attest to the excellent and compassionate care they have received. The Agency strongly believes that this care must be delivered within a working environment that reflects creativity and support and would welcome eager, experienced clinicians who would like to join this effort!
