Adult Transitional Care or Telemetry

There are 34 beds designated as transitional or telemetry beds. These beds are all located on the Parnassus campus:

  • Neurotransitional Care Unit
The Neurotransitional Care Unit is a 13-bed unit that is jointly managed by the Department of Medicine and the Department of Nursing. This unit provides state-of-the-art care to the acutely ill neurology or neurosurgical patient. It is located on the 8th floor of Moffitt Hospital.

The Neurotransitional Care Unit is equipped to care for a wide variety of neuro related disorders. General categories of disorders include central nervous system (CNS) tumors, CNS infections, cerebrovascular disorders (these involve blood vessels of brain, such as stroke), and some head, neck, or spinal injuries. The Neurotransitional Care Unit is not a neuro trauma unit. For further information about neurological problems, research, and clinical programs at UCSF, go to Medical UCSF Links.

The patient’s attending physician is responsible for directing and supervising the patient’s care. The patient’s nurse provides direct care and is responsible for continuous patient monitoring.

Our staff members recognize each patient as a whole individual and provide not only physical care, but emotional support and social services as well. We appreciate ethnic and cultural diversity and are sensitive to the concerns of the patient's family and loved ones.

  • Transitional Cardiac Care
The Transitional Cardiac Care is an acute care unit with nine beds. It provides care for adult hospitalized patients who require more frequent assessments and more complex care than the Cardiac Unit provides but who do not require placement in the Intensive Cardiac Care unit. All three cardiac units are located on the 10th floor of the adjacent Long and Moffitt Hospitals.

Patients in the Transitional Cardiac Care unit include those that require respiratory and specific hemodynamic monitoring and care. A wide range of medical disorders are represented including congestive heart failure, pulmonary hypertension (raised blood pressure in blood vessels supplying the lungs), myocardial infarction (type of heart attack) and arrhythmias (abnormal heart rhythm). Postoperative care is also provided.

The unit uses a team approach to patient care. The patient's attending physician is responsible for directing and supervising the patients' care. Nurses are responsible for ongoing patient assessment and the implementation of the physician orders - for more information see Transitional Cardiac Care Nursing. Other members of the patient care team include professionals in the areas of social work, pharmacy, dietary, physical, respiratory, speech and occupational therapy, and pastoral care.

  • Cardiovascular Telemetry

Located on the 10th floor of Long Hospital, this unit consists of 36 beds and serves inpatients admitted under the Cardiology Service; Vascular Surgery; and Cardiothoracic Surgery, including the Heart & Lung Transplant Service. Additionally, patients recovering from Interventional Cardiology procedures and Electrophysiology studies are admitted on this unit. The staff on 10Long strive to “Care from the Heart” and to provide an individualized approach to patient care, emphasizing patient and family education regarding lifestyle changes, diet modifications, and medications. This is a fast-paced unit involving multiple tests & procedures scheduled for patients, in addition to a moderately high number discharges and admissions per day. Because of this, teamwork is crucial to daily operations, and it is greatly promoted amongst the staff and other members of the health care team.

  • Medical / Surgical Telemetry
This unit, located on the 14th floor of Moffitt Hospital, has a patient population mix which includes transitional care devoted to medical and surgical patients and care devoted to non-transitional medical/surgical patients. Most of the beds have telemetry capability. High acuity, telemetry and general med-surg population make this unit a challenging and rewarding place to work.
