The 2007 Distinguished Nurse Award
Julia Chinn, Staff Nurse III on 12 Long, received the 2007 Distinguished Nurse Award for her contributions and leadership as a clinical expert.
Her advocacy for all patients, particularly those in the older population, is an indication of her care and dedication to nursing. After passing the national certification exam in geriatrics, Julia recognized the link to poor patient outcomes such as falls and the use of restraints in older patients with cognitive impairment.
Her interest in keeping patients safe led her to become one of the first Patient Safety Fellows in the department and prompted her to embark on an evidence-based practice project titled “Improving Care of Delirious Patients” under the mentorship of the geriatric CNS, Carla Graf.
Her work has led to the standardization of the assessment and management of acute cognitive changes and delirium in older adult patients. Bedside nurses are now provided with education and training on how to positively screen for and identify acute cognitive changes, implement a care plan and make interventions to prevent harm, and communicate their findings during patient hand offs. Julia has also implemented the use of the UCSF Delirium Care Plan and Falls Care Plan on all patients who are identified with delirium. She continues to mentor staff and refine her project as she follows patient outcomes and continually demonstrates a staunch commitment to all of her patients and to her goal of improving care of older adult patients.
